Mu-opioid receptor modulation and glutamate reuptake enhancement.
Most antidepressants require 2–4 weeks to produce noticeable improvement, with full effect at 6–8 weeks.
Never stop antidepressants abruptly — discontinuation syndrome (dizziness, flu-like symptoms, electric shock sensations) can occur. Taper gradually over weeks under physician supervision. SSRIs/SNRIs and paroxetine particularly require slow tapering. TCAs should be reduced slowly.
SSRIs and many TCAs cause sexual dysfunction (reduced libido, delayed orgasm) in 30–60% of patients — a major cause of non-adherence. Bupropion is uniquely associated with minimal or no sexual dysfunction. Mirtazapine, tianeptine, and agomelatine also have lower sexual dysfunction rates.
Tianeptine at high doses has opioid-like abuse potential; use at prescribed doses only.
Alcohol and antidepressants/anxiolytics interact unfavourably — increased CNS depression, worsened mood, reduced efficacy. Alcohol should be minimised or avoided during treatment with any psychotropic medication.
