Dapoxetine is the only medication specifically approved for premature ejaculation. Unlike daily SSRIs used off-label, dapoxetine is taken on-demand 1–3 hours before sexual activity and clears the body within 24 hours. In Phase III trials (n=6,081), dapoxetine 60mg increased ejaculatory time 3.5-fold and significantly improved sexual satisfaction. Avoid alcohol and do not drive for 24 hours after taking.

Take tadalafil 30–60 minutes before (daily version: any time). Sildenafil/vardenafil: 30–60 minutes. Dapoxetine: 1–3 hours. Sublingual/jelly: 15–20 minutes.
PDE5 inhibitors are absolutely contraindicated with nitrates (GTN spray, isosorbide). Caution with alpha-blockers and antihypertensives — additional blood pressure lowering effect. Dapoxetine: avoid MAO inhibitors. Always disclose all medications.
No — PDE5 inhibitors require sexual stimulation to produce an erection. The medication creates conditions for an erection but does not cause one independently.
Headache (most common), facial flushing, nasal congestion, dyspepsia. Sildenafil may cause visual tinge. Tadalafil may cause back pain/myalgia. Dapoxetine causes nausea and dizziness. Side effects are dose-dependent.
PDE5 inhibitors are approved for adults 18+. In younger men (under 40), ED is more commonly psychological — a physician should assess underlying causes before prescribing ED medication.
