What Is Prasugrel 10mg Used For?

Prasugrel 10mg tablets are used in patients with acute coronary syndrome (ACS) who are undergoing percutaneous coronary intervention (PCI — coronary angioplasty with or without stenting). Approved indications vary by country and regulatory authority.

United States: Effient® (prasugrel) is FDA-approved, in combination with aspirin, to reduce the rate of thrombotic cardiovascular events (including stent thrombosis) in patients with ACS — unstable angina (UA), non-ST-elevation myocardial infarction (NSTEMI) or ST-elevation myocardial infarction (STEMI) — who are managed with PCI. The standard maintenance dose is prasugrel 10mg once daily following a 60mg loading dose.

United Kingdom / EU: Efient® (prasugrel) is EMA-approved, in combination with aspirin, for the prevention of atherothrombotic events in adult patients with ACS undergoing primary or delayed PCI. The 10mg maintenance dose is used in patients not meeting criteria for the reduced 5mg dose.

Australia: Prasugrel 10mg Australia — Effient® and generic prasugrel 10mg products are subject to TGA registration and ARTG listing. PBS subsidisation depends on indication and eligibility — confirm with your prescriber or pharmacist.

UAE / Gulf: Prasugrel products are subject to UAE Ministry of Health registration. Consult a UAE-licensed physician or pharmacist for current prasugrel 10mg UAE availability.

Germany: Prasugrel 10mg is available in Germany as Efient® and as generic prasugrel 10mg Filmtabletten from multiple manufacturers. All are prescription-only (rezeptpflichtig).

France: Prasugrel 10mg is available in France as Efient® and as generic prasugrel 10mg comprimés pelliculés. Subject to prescription and HAS reimbursement rules.

Russia: Прасугрел 10 мг is available in Russia as Эффиент® and under generic names. All require a valid Russian prescription.

South Korea: 프라수그렐 10mg is registered for prescription use in South Korea per MFDS-approved labeling.

Always follow the prescribing information applicable to your country and the guidance of your licensed healthcare professional.

How Prasugrel 10mg Works

Prasugrel is a thienopyridine prodrug and P2Y12 receptor inhibitor. After oral administration, prasugrel is rapidly converted to its active metabolite in the body. The active metabolite irreversibly binds to the P2Y12 receptor on platelets, blocking ADP-mediated platelet activation and aggregation for the lifetime of the platelet (7–10 days).

Key pharmacological features of prasugrel vs other P2Y12 inhibitors:

  • Faster onset than clopidogrel — active metabolite is generated more efficiently, resulting in faster and more consistent platelet inhibition
  • Greater platelet inhibition than clopidogrel — less inter-patient variability due to more efficient conversion to active metabolite
  • Irreversible binding — like clopidogrel, prasugrel irreversibly inhibits P2Y12 receptors for the platelet’s lifetime
  • More potent than clopidogrel at standard doses — associated with greater reduction in ischaemic events but also greater bleeding risk

Prasugrel 10mg vs 5mg — What Is the Difference?

Both strengths contain prasugrel as the active ingredient. The difference is the maintenance dose:

  • Prasugrel 10mg is the standard maintenance dose for most patients following ACS with PCI
  • Prasugrel 5mg is the lower maintenance dose recommended for patients who are:
    • Body weight below 60kg
    • Aged 75 years or older (where prasugrel use is considered necessary despite increased risk)

The loading dose is 60mg regardless of maintenance dose. Dose selection is made by the prescribing physician based on patient weight, age and bleeding risk.

Do not change your prasugrel dose without guidance from your prescriber.

Generic Prasugrel 10mg vs Effient®/Efient® 10mg

Effient® 10mg (USA) / Efient® 10mg (UK/EU) is the originator brand containing prasugrel 10mg. Generic prasugrel 10mg tablets contain the same active ingredient at the same strength but may differ in:

  • Inactive ingredients and excipients
  • Tablet colour, shape and film coating
  • Packaging and blister configuration
  • Manufacturer and country of manufacture

Generic products are not automatically interchangeable with Effient®/Efient® in every market — local substitution rules apply. Consult your pharmacist before switching between branded and generic prasugrel products.

Manufacturers of generic prasugrel 10mg available internationally include products from Accord, Teva, Sandoz, Ratiopharm, Aurobindo, Sun Pharma and others depending on market.

Key Safety Information — Prasugrel 10mg

Bleeding Risk — Most Important Risk

The most significant safety concern with prasugrel 10mg is serious and potentially fatal bleeding. Prasugrel inhibits platelet function irreversibly, increasing the risk of bleeding from all sites including surgical and procedural sites, gastrointestinal tract and intracranial bleeding.

Patients at highest bleeding risk include those who are:

  • Aged 75 years or older
  • Body weight below 60kg
  • Having urgent surgery (especially CABG)
  • Taking other medicines that increase bleeding risk (anticoagulants, NSAIDs, SSRIs)

Contraindications
Prasugrel 10mg is absolutely contraindicated in patients with:

  • Prior stroke or transient ischaemic attack (TIA) — this is a black box warning in the USA
  • Active pathological bleeding
  • Severe hepatic impairment
  • Known hypersensitivity to prasugrel or any excipient

Black Box Warning (USA)
Prasugrel carries an FDA black box warning for serious bleeding risk. It is contraindicated in patients with prior stroke or TIA due to net clinical harm in these patients in the TRITON-TIMI 38 trial.

Discontinuation Before Surgery
Prasugrel should be discontinued at least 7 days before elective surgery where bleeding risk is a concern, including CABG. Patients must not stop prasugrel without guidance from their cardiologist — premature discontinuation increases the risk of stent thrombosis and myocardial infarction.

Drug Interactions
Significant interactions include:

  • Warfarin and other anticoagulants — increased bleeding risk
  • NSAIDs (chronic use) — increased bleeding risk
  • SSRIs and SNRIs — increased bleeding risk
  • Strong CYP3A4 inhibitors — limited effect on prasugrel (unlike clopidogrel)
  • Opioids — may delay absorption of prasugrel active metabolite in STEMI patients

Always read the patient information leaflet (PIL) and prescribing information applicable to your country before taking prasugrel.

Frequently Asked Questions — Prasugrel 10mg

What is Prasugrel 10mg used for?
Prasugrel 10mg is used in combination with aspirin to reduce the risk of heart attack, stroke and cardiovascular death in patients with acute coronary syndrome (ACS) who are undergoing percutaneous coronary intervention (PCI). It prevents blood clots from forming in coronary arteries and stents.

Is Prasugrel 10mg the generic for Effient® / Efient® 10mg?
Yes. Effient® (USA) and Efient® (UK/EU) are the originator brands containing prasugrel 10mg. Generic prasugrel 10mg tablets contain the same active ingredient and strength. Inactive ingredients, appearance and manufacturer may differ.

What is the difference between Prasugrel 5mg and 10mg?
Prasugrel 10mg is the standard maintenance dose for most ACS/PCI patients. Prasugrel 5mg is the reduced maintenance dose recommended for patients weighing less than 60kg or aged 75 years or older. The loading dose is 60mg for both. Dose selection is made by the prescribing physician.

How does Prasugrel compare to Clopidogrel?
Prasugrel is a more potent P2Y12 inhibitor than clopidogrel with faster onset and less variability in response. It reduces ischaemic events more effectively in ACS/PCI patients but carries a higher bleeding risk. Prasugrel is contraindicated in patients with prior stroke or TIA, where clopidogrel remains an option.

Can Prasugrel 10mg be stopped before surgery?
Prasugrel should be discontinued at least 7 days before elective surgery. Never stop prasugrel without consulting your cardiologist — stopping prematurely can cause stent thrombosis, which is life-threatening.

Does Prasugrel 10mg increase bleeding risk?
Yes. Prasugrel carries a significant bleeding risk. It is contraindicated in patients with prior stroke or TIA (black box warning in the USA) and in those with active bleeding. Patients aged 75 or older or weighing under 60kg have a higher bleeding risk on the 10mg dose.

Can I buy Prasugrel 10mg tablets online?
Prasugrel 10mg is prescription-only in the USA, UK, Australia, UAE and most markets worldwide. A valid prescription from a licensed prescriber is required.

Is Prasugrel 10mg available in Australia on the PBS?
Prasugrel 10mg products are registered on the ARTG. PBS subsidisation depends on indication and patient eligibility. Confirm current PBS status with your prescriber or pharmacist.

What is Prasugrel 10mg in Germany?
Prasugrel 10mg Filmtabletten are available in Germany as Efient® and as generic prasugrel from manufacturers including Accord, Teva and Sandoz. All are prescription-only (rezeptpflichtig).

Is Prasugrel the same as Clopidogrel or Ticagrelor?
No. Prasugrel, clopidogrel and ticagrelor are all P2Y12 inhibitors but they differ in chemical class, potency, onset, binding characteristics, contraindications and approved indications. They are not interchangeable without medical guidance.
